Pharmacist - Casual
We're seeking a skilled, clinical staff pharmacist to join our team! In this casual role, you'll work variable hours to assist in covering scheduling gaps.
Clinical Responsibilities:
Interprets and clarifies medication orders and applicable treatments directly from the chart or chart copy.
Maintains patient records to monitor drug therapy for appropriateness and safety.
Is responsible for assuring that medication schedules are appropriate in relation to the patient's ability to metabolize and eliminate specific drug compounds (i.e., Pharmacokinetic Consultation)
Assists the nurse with the routine updating of the patient's medication administration record (MAR) as appropriate.
Provides drug information for medical, nursing or other hospital professional staff.
Participates in the selection of appropriate medication(s) for specific indications.
Confers with patients and Medical Staff as required and works to advance the practice of Clinical Pharmacy.
Drug Distribution Responsibilities:
Dispenses and compounds or supervises the dispensing of medications and intravenous admixtures.
Compounds medications for floor use and hospital Department of Pharmacy Services use.
Interprets doctors orders and creates a patient drug profile.
Supervises the sterile compounding of all pharmaceuticals prepared by the Department of Pharmacy Services.
Inspects satellite pharmacy and/or medication storage areas on patient floors and nursing areas.
Maintains records on controlled drugs.
Supervises pharmacy technicians, pharmacy clerks and pharmacy interns.
Maintains pricing and location index of pharmacy drug formulary and monitors the preparation of accurate patient charges (i.e., billing).
Other duties as assigned in compliance with all departmental and hospital policies and procedures.
Teaching Responsibilities:
Serves as an educator of hospital staff and students by preparation of clinical conferences and/or "inservice."
Participates in educational programs by serving as a preceptor to externship and/or clerkship students and by providing lectures
Administrative Responsibilities:
Participates in appropriate hospital committees to which the individual is assigned (i.e., P&T, Patient Safety, Performance Improvement, etc.).
Develops and/or revises interdepartmental policies and procedures as requested
Implements decisions of the Pharmacy and Therapeutics Committee.
Works with the Medical Staff to establish a formulary system; evaluates and reviews medications for addition or deletion to or from the Hospital Formulary.
Work/Education requirements:
Registered pharmacist in the state of Pennsylvania
Familiar with unit dose, IV admixtures and clinical pharmacy practice
Prior hospital experience preferred
